I'll personally admit that there have been times when I've told my art director something like "I know the book says this, but that would look stupid -- so tell the artist to do it this other way." I'm not ashamed of it, either.

I'm not much bothered either. I don't assume that the cover is going to be a mental image that is conjured up while reading the book. Mind you I used to work in publishing and I dealt with a lot of art so I know how divergents things can get. But I think I had that attitude even before that experience.
